Hi I am Eloisa,

I am a nurse with more than three decades of experience caring for others, including 17 years dedicated to Home Health. Throughout my career, I have had the privilege of walking alongside patients and families during some of life's most vulnerable and important seasons. Those years of hands-on care have shaped not only my professional experience, but also my heart for serving seniors with compassion, dignity, and understanding.

Working in the medical field taught me that true care goes far beyond medical needs. It's about listening, building trust, offering comfort, and helping families feel supported during times of uncertainty and transition.

Being welcomed into people's homes and lives has given me a deep appreciation for the emotional, physical, and spiritual needs that come with aging and change.

Today, I bring that same compassion and experience into my role as a Senior Transition Specialist. My nursing background allows me to better understand the unique challenges seniors face, while helping families navigate important decisions with confidence and peace of mind. I strive to make every family feel heard, cared for, and never alone in the process.

Those who know me often describe me as compassionate, patient, trustworthy, and someone who genuinely cares beyond the job itself. i believe every senior deserves to feel valued, safe, and supported in this chapter of life, and it is truly an honor to help families find the best path forward for their loved ones.
